How to access South Western Sydney HealthPathways
If you are a health professional in South Western Sydney and would like to have access to the South Western Sydney HealthPathways website, please contact the South Western Sydney HealthPathways Team who will provide you with a username and password. The Team can be contacted via email or telephone 0455 052 140 (Monday to Friday).
What is HealthPathways?
HealthPathways is a web-based information portal supporting primary care clinicians to plan patient care through primary, community and secondary health care systems within South Western Sydney. It is like a 'care map', so that all members of a health care team – whether they work in a hospital or the community - can be on the same page when it comes to looking after a particular person.
HealthPathways are designed to be used at the point of care, primarily for General Practitioners but is also available to Hospital Specialists, Nurses, Allied Health and other Health Professionals within South Western Sydney.

Please note that this site contains two types of pathways:
- Localised pathways for the care and management of patients within the South Western Sydney Region, have been developed jointly by local general practitioners, specialists and other health care providers.
- Canterbury Initiative (New Zealand) pathways are pathways developed by the Canterbury Initiative (New Zealand) and have been developed jointly by NZ primary and secondary care clinicians for care of people in the Canterbury Region. These pathways are identified by having a light orange background. The information in the Canterbury Initiative pathways have not yet been localised and there is some content that will not be relevant to the local South Western Sydney Region.
The pathways are designed for use by all clinicians with an emphasis on primary care. They are not designed for access or use by non-regulated health service providers or patients.
